About Us

Home2Home Transition Services, LLC is a privately-owned Wisconsin company established in 2007 that serves Milwaukee, Waukesha and S.E. Wisconsin.

Home2Home Transition Services Minnesota, LLC is a privately-owned Minnesota company serving the Twin Cities and surrounding areas. 

Both locations assist senior citizens and people of any age with the difficult job of downsizing and/or moving.  (See “Services”)

In Wisconsin

Pat Mattiacci, founder and owner of Home2Home Transition Services, LLC , is a long-time resident of the Waukesha, Wisconsin area. She has enjoyed the customer service experience for many years, developed primarily in interior design, retail, and education. Pat and her husband have owned a number of homes, so that experience combined with her natural organization skills laid a firm foundation for this business. She has also enjoyed her work as a Caregiver for senior citizens with an in-home senior care company.

In 2007, Pat successfully completed the Senior Move Management training course provided by Smooth Transitions® and Barbara H. Morris in Louisville, Kentucky. Ms. Morris is a Founding Member of the National Association of Senior Move Managers and has trained many new move managers. Pat has continued her education by attending courses presented at NASMM’s annual conferences, including the 2010 conference in Las Vegas.

Pat attended the University of Wisconsin-Stevens Point majoring in interior design, and she recently earned the Customer Service Representative certification from Rockhurst University Continuing Education Center, Inc. She is a member of the National Association of Senior Move Managers, the SeniorCare Resource Group of WI, Inc., the Waukesha County Senior Concerns Consortium, and the North Shore Senior Resource Network.

Home2Home was accredited by the Better Business Bureau of Wisconsin in 2009.


In Minnesota

The owners of Home2Home’s Transition Services Minnesota, LLC, Jenny Mattiacci and Kim Jaeger, are long-time residents of Minnetonka. Jenny spent 10 years working in privately owned student housing at the University of Minnesota where she coordinated the move in/outs for thousands of college students.  For the past 10 years, Jenny has been a sales representative in the home accessory industry working closely with furniture retailers and interior designers. She also currently works part-time for a local Realtor.

Jenny has earned a Certificate in Senior Move Management (cSMM) from eSMMart. The training course was developed by the Founding President of the National Association of Senior Move Managers, Margit Novack.

Kim spent 10 years as a bookkeeper in the construction industry.  Kim’s interests and hobbies include remodeling, organizing and interior design.  Both Kim and Jenny are actively involved in the PTA, church and their community.

Jenny and Kim attended courses at NASMM’s 2010 annual conference, and those included: More Than Enough Stuff:  Chronic Disorganization, Hoarding and Senior Move Managers; Vital Record and Document Organizing for Seniors; and Business Beyond Move Management:  Redesign and Home Staging.

The Mission of Home2Home Transitions is to simplify and expedite our client’s transition to a new home by providing individualized downsizing, moving and organizing services in a compassionate, confidential, efficient and professional manner.

Our Promise is to listen to the needs of each client and their family, to respect each client’s priorities, and to tailor our services to each client’s unique situation.

Our Policies for Doing Business

CONFIDENTIALITY is of utmost importance. We recognize that our job requires our clients to put their trust—as well as their stuff—in our hands. We will respect your privacy and the privacy of your family. We will arrive at your home without judgment and leave without any information.

THE COST OF OUR SERVICES is typically figured on a per-hour basis for each Home2Home staff member.  There may be instances where it is more appropriate for us to offer a service or services for a flat fee; each client’s situation will be assessed individually so that we can provide the best value for their investment.

EMPLOYEES of Home2Home Transition Services, LLC are carefully chosen and undergo a complete background check before they are hired.  Their work is covered by our Liability Insurance, and they are fully covered by Workers Compensation Insurance.

FAMILY AND FRIENDS can provide crucial support to our clients during their transition.  If they are available to offer assistance to our client, we will do our best to work with them to provide seamless, efficient assistance to our client.

Our policy is to REDUCE, REUSE & RECYCLE whenever possible in regards to belongings that are no longer needed by our clients.  We will follow each community’s trash pick-up and recycling guidelines, and we will properly dispose of any hazardous waste materials.  

REFERENCES are available upon request from past clients, senior living residences, and other senior service companies.
